The following forms should be printed, filled out and brought with you on the day of your visit. Please select the form based on the exam you will receive.
Depending on your scheduled procedure, you may be with us for 30 minutes or a few hours. Regardless, we’re committed to making your time with us as care free and relaxing as possible.
Exam Day
First and foremost, please be sure to follow any instructions that are special to your exam. For more information on the exam you may be having and to find a list of Frequently Asked Questions, please click on to Our Services.
So that we may more efficiently register you prior to your exam, please…
- Arrive 15 or more minutes prior to your scheduled appointment time
- Have a valid drivers license and a valid insurance card with you (for a list of accepted insurances, click here)
- Bring the exam order from your doctor
- Bring completed patient registration forms and history forms with you
- Leave valuable jewelry at home!

For longer exams or for support, we encourage you to bring a family member or friend with you. They can wait for you and accompany you home after your exam.
Special Note: For the comfort and safety of all, please find alternative care for children. Although we all love children, we cannot be responsible for their safety or whereabouts during your exam.
